I actually thought it might be a heart from one of those packs of innards included with frozen or fresh birds. That is the most matter-of-fact use of innards to express a loving message that I have ever seen.

If it was a piece of excrement, all you need is to add the word “not” at the end of the sentence and it would be a more generally usable rebus.
