Was it big boy rules?
So, one of those guys that posts training videos on the internet that gun blogs link to and point out the suck and fail? Yeah, he went and got himself shot:
Unknown to him [Sonny Puzikas], one of his instructors was still inside when he opened fire with a 9 mm semi-automatic pistol, striking unaware instructor in the hand and twice in the abdomen.

Actually, I think it was the video instructor that everyone points at a laughs is the one that did the shooting and shot another instructor who was just doing his job as he was supposed to. Much Worse.

“big boys rules” is when some mall ninja wannabe running a training class violates the snot out of the four rules, while claiming that it is OK because real operators play fast and loose with safety rules while doing “realistic” training.
